# Prompt: create your own one-page decision sheet

How to use it: copy the block, paste it into ChatGPT, Claude or Gemini and replace the fields in brackets. Pick a decision that keeps coming back in your area: approving an exception, renewing a vendor. You get a sheet in this hub's format. Test it on two real decisions before you share it. Do not paste confidential data unless your company has approved it.

```text
Act as a designer of management tools who turns recurring decisions into one-page sheets. A good sheet forces people to write down the criterion, the owner, the date and what would change the decision.
Context I am giving you:
- Company and function: [e.g. distribution, 500 people, procurement]
- Recurring decision: [e.g. "approve or reject a new vendor"]
- How often it happens and who makes it: [ ]
- What goes wrong today when it is made: [e.g. approved without comparison, nobody reviews it afterwards]
- Information usually available: [ ]
What I need: a new sheet with exactly this structure, in this order:
1. Title: one sentence that states what the sheet delivers, not the topic.
2. One line under the title: who it is for and when it is used.
3. When to use it: 3 bullets with concrete situations from my area.
4. The sheet: a table of 6 to 10 rows with columns Field | What to write | Illustrative example. Always include an owner field, a review date field and a "what would change my mind" field.
5. How to fill it in N minutes: 4 to 6 numbered steps; one of them names the mistake almost everyone makes.
6. The line that is almost always missing: a short paragraph on the field people forget and why it matters.
Output format: Markdown, maximum 500 words, fitting on one printed page. A single table.
Rules:
- Every numerical example is made up for illustration: label it "illustrative". Do not present any figure as real.
- Do not invent regulations, studies or statistics. If a field needs data I do not have, write [DATA PENDING].
- The decision and the criterion belong to a named person, not to the AI.
- No sales language, no urgency, no return promises.
- If critical information is missing, ask me at most 3 questions before answering.
- Answer in plain, direct business English, short sentences, no emojis.
```

What to check in the answer:

- Does the table include owner, review date and "what would change my mind" fields? Without them, it is just a form.
- Does every example say "illustrative"?
- Does it fit on one page? If not, cut rows.

If one day the decision is hard to reverse and carries your name, the free tools at vicsol.org/decision-clarity/ help you structure it. They do not replace a formal review.
